If a safety-critical computerization project didn’t have Level 4 automated testing, would this be clear?

evidence that the project had been sabotaged from within ?

What would possess an organization responsible for safety-critical computerized systems not to have “Level 4?

automated testing” ?

Would you have used this method, which seems to have been industry-standard, to set to work Collins Class?

submarine computerization software ?

Would there have been a formal validation plan for Collins ? (Surely there was ?) Was it ever used?

It’s a bit rich telling the supplier his system doesn’t work, if the formal validation plan was never followed.

Would this help, to deploy a good system ? What if people treat the document as a checklist of what not to do?
